The information on this page comes from the MN Public School District 581 School Nurse. Please read all of the information carefully. If you want your student to participate, print the Consent form, fill it out (one for each student) and bring it to the school office.  When the vaccine is available your child will receive it.  You can withdraw your consent at any time.

2009 H1N1 INFLUENZA VACCINE - This is an informational sheet distributed by the Dept. of Health and Human Services Centers for Disease Control and Prevention (CDC).

2009 H1N1 INFLUENZA VACCINE CONSENT FORM - This is the form you need to print and fill out if you want your child to receive the vaccine at school.  This form must be on file at school before the vaccine will be given to your student.
